We take your feedback seriously.

We invite you to participate in this survey to measure your experience in our hospital. Your views are very important in helping us to identify things that need to be improved in our approach to patient care. Your feedback will also allow us to promote best practice and continuous improvement. The questionnaire should take approximately 15 minutes to complete.

All information you give will remain confidential and will not be used in decision-making about your treatment. You are free to withdraw at any time if you do not feel comfortable answering a question. Any reports about survey results will remove personal patient information and will group findings to show overall patterns, not individual responses.
